INSURANCE INFORMATION

Services offered at Mindful Maelstrom Wellness Clinic are not covered by OHIP. However, services may be covered by private extended health benefit plans. Please check your benefits before booking to confirm your coverage. This is specifically important for Psychotherapy, as plans will vary. Invoices will be sent to you by email to be remitted to your provider. 

 

DIRECT BILLING

Direct billing is available for Registered Massage TherapyAcupuncture, and Naturopathic Medicine for the following insurance providers: Canada Life, Manulife, Sun Life, LiUNA Local 183, Chamber of Commerce, Industrial Alliance and Green Shield. Please provide your insurance card before your appointment. You will be responsible for any outstanding, rejected or co-pay amounts. Unfortunately we are not able to direct bill for other modalities at this time. 

 

SLIDING SCALE / REDUCED RATES

We hold a limited number of Psychotherapy spots for those who are uninsured and unable to pay the full session fee. A rate reduction application form must be completed with proof of limited income. The maximum number of sessions a client can attend at a reduced rate is 12. This is to ensure spots open up on a regular basis for those in need. PAYMENT POLICY

Payment is due at the time of your visit. Accepted methods of payment are cash, credit card, Interac debit or e-transfer. Non-payment or a history of late cancellations or no-shows may require the clinic to collect a deposit or credit card number to be kept on file for future bookings. Outstanding invoices must be paid prior to subsequent visits. 

CANCELLATION POLICY

Mindful Maelstrom requires a minimum of 24 hours to cancel or reschedule an appointment. If you are unable to provide sufficient notice or if your appointment is missed, the full appointment fee will be applied. You may cancel or reschedule your appointment by phone or email during business hours, or through your online account until 24 hours in advance. Cancellation fees are not covered by private insurance.

We understand that circumstances can arise that are outside your control. To be fair to other clients who were not able to book in your reserved time, and to our practitioners who have held this time for you, we apply our cancellation policy automatically and without bias.
